Access for Justice Releases New Form for Public Comment

Request for Name Change (Minor Children)

The Illinois Supreme Court’s Commission on Access to Justice is pleased to announce that the Request for Name Change (Minor Children) form suite is now available for public comment. The draft forms can be found here: http://www.illinoiscourts.gov/Forms/drafts/default.asp. The public comment period is 45 days.  After that time, the Commission will review any feedback or suggestions received and make any necessary revisions.
